BSW Upper Division Pre-Admission Form

Please note, you must have your UofL student ID# to proceed. 

In order to proceed with your admission to the program, please fill out this form and click the submit button to complete the task. Kent School will use your information to setup your account so you can complete the BSW Upper Division application.

If you do not have a university issued student id#, you must complete the application for admission to the University of Louisville. Apply now and indicate social work as your major on the UofL application. Once admitted by the Admissions Office, you will receive a student id#, and can now complete the BSW Upper Division Pre-Admission Form.
